Now I have organised and arranged models for my film, I'm able to create a risk assessment for taking equipment from SiSo. Below is the 4 page risk assessment I filled out online. This process took about half hour, to fill in all the boxes, get locations, phone numbers and double checking all of the information. As for the equipment I'm taking out, I will make a separate blog post talking about that.
I filled out the risk assessment as accurate as I could with the planning and information I had already for shooting. This means that there are some variables that can change when it actually comes to shooting, such as locations, there could be last minute changes to work with the models busy schedules. Or one of my models could drop out of the project, meaning I would have to find someone else... Considering these things could be altered, I still filled this out as accurate as I could. I also added some details of Alesha's film as she will be using the camera for a bit when I take it out, it was important I added her details as well about her filming as this could affect the risks.
Information in a risk assessment include:
Title
Activity description
People at risk (name, role, contact)
Equipment being used (where is it being stored?)
Locations (& date)
Emergency procedures
Potential risks and activities
Hazards (who's at risk?, control measure...)




This process was difficult as I had never done something like this before, but know I have done it once I feel like when I need to get any other equipment out at the University I will be able to do so confidentially. It is also helpful for myself to evaluate all the risks/hazards of my film and what I'm creating.
